- We will submit it to the U.S Department of State the next business day for processing.

Personal Appearance 

- Both Parents and child will visit the local passport agency/office.

 

- A federal agent will review the documents and instruct you to verify and sign your application.

 

- The federal agent will then seal the envelope and hand it back to you. /office.

Receive Your Passport

- Once your passport has been issued we will contact you immediately! You may either pick up your new passport or have it shipped back to you.

A Few Things to Know, Before You Start...

  • When applying for a U.S. passport for a child under 16, both parents or legal guardians are generally required to be present with the child at the time of application. This rule is in place to help prevent international child abduction and ensure proper consent from both guardians. If one parent cannot appear, additional documentation such as a notarized consent form (Form DS-3053) may be required.

  • Child passports are only valid for five years, not ten like adult passports. Because children grow and change quickly, a new application is required each time the passport expires.

  • Carefully reviewing all documentation before the appointment is important, as missing parental consent, incorrect forms, or incomplete paperwork are common reasons for delays.

Required Documents For minor U.S. Passport (Under 16 Years Old)

1- Old Passport Book and Old Passport Card (If Applicable)

  • MUST provide your old passport (If Applicable).

3- Required Documents

  • MUST provide your original birth certificate, certified copy or original naturalization certificate.

  • MUST provide Name Change documentation, if applicable (Original marriage certificate OR court order documentation) 

4- Parents or Guardians Identification

  • BOTH parents must provide Photo copies (BACK and FRONT) of driver’s license, federal or state issued ID, or passport.

5- Travel Itinerary

  • Must be a round trip itinerary printed from an online vendor or on travel agency letterhead.

6- Passport Type Photos

  • Two 2x2 passport type photographs

  • Photo MUST be taken within the past 6 months 

  • Neutral expression and photo must be in high quality 

7- Additional Required Forms

8-  Consent Form DS-3053

  • Both parents or guardians must accompany the minor to the Passport agency. IF one of the parents is unable to be present, a notarized consent form must be submitted by the parent or guardian that will be present (Issued within 3 months). Please CLICK HERE to print and fill out the consent form.
