Ship/Drop Envelope
- You may either ship in or drop off your envelope at our location with an appointment.
- We will submit it to the U.S Department of State the next business day for processing.
04.
Personal Appearance
- You will visit your local passport agency/post office.
- A federal agent will review the documents and instruct you to verify and sign your application.
- The federal agent will then seal the envelope and hand it back to you./post office.
05.
Receive Your Passport
- Once your passport has been issued we will contact you immediately! You may either pick up your new passport or have it shipped back to you.

1- UNSIGNED DS-11 Form

2- Required Documents

MUST provide your original birth certificate, certified copy or original naturalization certificate.

MUST provide Name Change documentation, if applicable (Original marriage certificate OR court order documentation)

IF you cannot obtain one of these required documents, The Department of State can do a FILE SEARCH service for $150.
3- Proof of Identification

MUST provide your VALID driver’s license or Federal State ID card

IF you do not obtain an ID card:

You must visit your local DMV and apply for either a driver license or State ID card. Please provide the DMV temporary print out as proof that you have applied for an ID.

REQUIRED WITNESS: During your personal appearance at the Post Office you must bring a witness you have known for over 5 years and can provide proof of identification. Witness can be a friend, family member, but cannot be a co-worker or employer/employees. Please note: Your witness will sign a witness form.
4- Additional Required Forms

Must print 3 copies of the Authorization Form with ORIGINAL SIGNATURE.

5- Travel Itinerary

Must be a round trip itinerary printed from an online vendor or on a travel agency letterhead.
6- Passport Type Photos

Two 2x2 passport type photographs

Photo MUST be taken within the past 6 months

Neutral expression and photo must be in high quality
7- Payment

ONLY cash, check or ATM card is accepted at the acceptance agency (credit cards are NOT accepted)

Processing service fees may be paid by cash, check, debit/credit card and money orders directly to Sky Embassy. You may make the payment in person by visiting our office, by mail, or by phone.
