IMPORTANT INFORMATION TO COMPLETE YOUR ONLINE PASSPORT FORM

  • Application must be completed on a laptop or desktop and printed out to sign as it may not be supported on any other device.

  • NO hand-written forms can be accepted. The application form MUST be completed ONLINE and typed in all caps.

  •  SOCIAL SECURITY NUMBER must be included and correct as it can delay/reject passport issuance. 

  • Enter FULL NAMES including your middle names as it appears on your previous passport or legal documents. In failing to do so this will cause delays/rejection/suspension by the Department of State.

  • Do not abbreviate, do not include nicknames, or titles to your name. Name must appear identical as in previous passport/legal documents.

  • Must sign completed application. Original signature is required
